Job summary

Access Alliance Multicultural Health and Community Services in Toronto seeks a Community Action Coordinator to lead community engagement and outreach efforts at the east location and a Population Health Clinic. You will work with a multidisciplinary team to promote health equity and connect clients to essential health and social services.

The role requires a graduate degree in health sciences or related field and at least 3 years frontline experience in community engagement and program

Qualifications

• Minimum 3 years frontline experience in community engagement based program coordination, planning, implementation and evaluation.
• Experience working with low-income and priority groups.
• Graduate degree or equivalent combination of education and work experience.

Responsibilities

• Plan, implement and evaluate community engagement, peer outreach, and community ambassador initiatives.
• Identify health priorities with marginalized communities and develop strategies to address gaps.
• Collaborate with interdisciplinary teams to deliver health promotion and social support services.
