About The Opportunity

Reporting to the Manager of Patient Flow and Medical Social Work, the Medical Social Worker will provide social work services to patients and families, including support groups. The role requires independence, adaptability, and collaboration in various settings. Some home visits and travel are required.

About You

We are looking for candidates with:

• Master’s Degree in Social Work (candidates enrolled in a master's program will be considered; Bachelor of Social Work may also be considered if no other candidates qualify)
• Registered or eligible for registration with the Nova Scotia College of Social Workers (NSCSW)
• Minimum of 2 years clinical experience
• Extensive knowledge of community resources
• Ability to work independently and with multidisciplinary teams
• Experience in coordinating and documenting care plans for hospital discharge and community transition
• Innovative problem-solving skills and evidence-based practice application
• Knowledge of Joint Discharge Operations Committee (JDOC)
• Knowledge of other languages, especially French, is an asset

• Long-Assignment, Full-Time, 75 hours bi-weekly
• Approximately 12 months

Compensation and Benefits

$40.18 - $54.14 hourly. Successful candidates may be eligible for benefits including health, dental, travel, disability, life insurance, and a pension plan.
