Job summary

The EDiCare Physician at Vancouver General Hospital (VGH) is a locum, full-time role within the Vancouver Coastal Health geriatrics program in the Emergency Department. The physician leads discharge planning with acute care, community primary care and Home Health services, delivering patient- and family-centered care.

This role supports frail elders in the ED, collaborating with an interdisciplinary team to avoid hospital admissions and ensure safe, timely discharges.

Responsibilities

• Develop and implement safe discharge plans in collaboration with acute care services, community primary care physicians and Home Health services.
• Provide patient- and family-centered care.
• Support the Emergency Department in managing complex vulnerable geriatric patients to avoid unnecessary hospital admission.
• Develop collaborative trusting professional relationships with interdisciplinary team members, health authority staff and community physicians.
• Liaise with community primary care providers and specialists to support safe discharge in the Frail Elderly.
• Communicate clearly with patients, families, and other health care providers.
• Initiate and engage with patients and families in goals of care discussions.
• Provide medical oversight for home health nurses providing care in the community.
• Partner with Home and Community Health to promote team-based care to prevent ER visits and admissions.

The mandate of EDiCare follows the “Home is Best” philosophy for the Frail Elderly. Its goal is to redirect frail elders from a path of hospital admission towards discharge home when it can be safely facilitated with community and medical supports. The EDiCare Physician role presents a unique opportunity for physicians with a passion for our growing frail elderly population. The EDiCare Physician works as a member of a decade strong interdisciplinary team within the Emergency Dept at VGH with a focus on disposition planning. Frail Elders in the Emergency Department are identified and reviewed for appropriateness for a safe discharge home rather than a prolonged hospitalization, often associated with detrimental effects on a frail elder’s quality of life.As a Vancouver General Hospital (VGH) EDiCare Physician with Vancouver Coastal Health, you will:

• Provide medical oversight for home health nurses providing care for patients in the community
• Partner with Home and Community Health to promote team-based care to prevent unnecessary ER visits and admissions.
