Position Summary

Reporting to the Clinical Manager, Outpatient Mental Health and Addiction Services, the successful applicant will be working as a member of the Early Intervention (Walking Alongside Youth-WAY) interdisciplinary team serving youth and young adults aged 16-25 years who are experiencing, emerging and/or persistent mental health and addictions issues in Brant Haldimand and Norfolk regions. This role is responsible for mental health social work assessment, counselling, crisis management and system navigation when needed.

Education Requirements

  • Masters of Social Work Degree from an accredited University School of Social Work

Mandatory Qualifications

  • Current certificate of registration with the College of Social Service and Social Work

Mandatory Competencies

  • Experience with multiple modes of clinical intervention including but not limited to crisis, psychosocial assessment, CBT, DBT, Coordinated Care Planning, Trauma Informed approaches, Motivational Interviewing, Acceptance and Commitment Therapy and other evidenced based care approaches that can demonstrate a measurable impact on improving care and outcomes for youth and young adults.
  • Clinical competence and commitment to quality patient care
  • Demonstrated understanding and commitment to cultural competency related to Indigenous people and the related trauma from systemic racism and colonization.
  • Excellent understanding of the effects of intergenerational trauma and cultural based approaches to healing and evidenced based care.
  • Excellent communication/interpersonal skills
  • Excellent conflict resolution skills
  • Working knowledge of Recovery Model of Practice as outline by the Mental Health Commission of Canada
  • Competency to provide service to individuals with Concurrent Disorders
  • Evidence of ability to work in an interdisciplinary team
  • Excellent client assessment skills, including a mental status exam
  • Demonstrated commitment to continuing education
  • Ability to work with persons with Serious Mental Illness
  • Ability to provide therapeutic interventions in group context.
  • Must have good organizational skills to be able to cope with a fluctuating workload and competing demands.
  • Crisis assessment and intervention experience; solution-focused therapy
  • Knowledge of community resources.
  • Experience with program design and implementation and asset
  • Direct experience and capacity to work with youth including TSLGBTQ and gender fluid issues is considered an asset.
  • Experience in virtual care considered an asset.
  • Access to a vehicle is required to due to travel to multisite locations and outreach in Brant and Haldimand and Norfolk counties is expected in this role
  • Safety (patient, worker & workplace) is a BCHS Corporate Priority. The successful applicant will demonstrate good stewardship in the identification, reporting and mitigation of unsafe acts or conditions.

  • Reliable individual with excellent attendance record and able to work all shifts
