Qualifications

Job description

COMMUNITY SUPPORT WORKER

About The Role

As our Community Support Worker, you will work as part of a vibrant team to provide care and support to people living in staffed homes within their community. You’ll encourage and empower the people we support to be independent and self-determining and to live the life they envision while maintaining excellent standards of care, health, and safety. Your natural ability to care for others and work both independently and collaboratively as a member of a multidisciplinary team will allow you to influence your team’s success.

Core Responsibilities

  1. Ensure the supported persons’ rights and choices are respected and advocate as needed.
  2. Interact effectively with people we support.
  3. Empower the people we support to be full citizens of their community.
  4. Participate in the development, implementation, and documentation of Personal Service Plans.
  5. Foster positive relationships with persons’ family members, professionals, co-workers, and members of the community.
  6. Assist with activities of daily living and medication administration.
  7. Understand behaviour management principles and strategies, including mental health needs.
  8. Record daily activity and personal health information (PHI) of the people we support in our case management database.
  9. Work with the people we support to ensure their home is clean and well-maintained, including daily food preparation and light housekeeping duties.

You will have

  1. Grade 12 education.
  2. Community Support Worker Diploma or equivalent.
  3. Minimum 2 years of experience in community social services/health care.
  4. Current Emergency First Aid/CPR and Food Safe certificates.
  5. Previous behaviour management training.
  6. Effective communication skills (verbal & written) with team members, professionals, families, and community members.
  7. Non-verbal communication skills.
  8. Restricted Class 4 driver’s license (for Full-Time positions).
  9. Satisfactory driver’s abstract.
  10. Criminal record clearance for children and vulnerable adults.
  11. TB test.
  12. Moderate computer literacy.
  13. Eligibility to work in Canada.
