Qualifications

What is Pay Per Visit?

  • A competitive rate that is paid per client visit, rather than care code or care time
  • For longer visits or extra visits (exceeding 1hr 59 min), you will be paid an additional visit
  • This model allows you to work ahead in your day but also take the time you need, as you see fit
  • This model relies on you using your professional judgment for client care
  • FT status nurses will be guaranteed 70 visits biweekly, providing peace of mind with a guaranteed income

Responsibilities

  • Complete comprehensive client assessments, develop care plans, implement and evaluate care based on evidence and client’s needs.
  • When designated as a primary provider, you will lead care for your client caseload, collaborating with other team members and support functions.
  • Support clients through care transitions by connecting them with community support services.

Requirements

  • Current registration with the College of Nurses of Ontario
  • Canadian Nurses Association (CNA) specialty certification is an asset
  • Enthusiasm for community care, evidence-informed practice, and excellence in care
  • Current CPR certification
  • Valid driver’s license and vehicle (travel required)
  • Updated immunization records, clear criminal background and Vulnerable Sector checks upon hire
  • Willingness to attain Saint Elizabeth certifications
  • Availability for day, evening, night shifts, and every other weekend
  • This role is physically demanding; requires dexterity, sensory ability, lifting up to 55 pounds, walking, climbing stairs, standing, bending, reaching, and movement
