Job summary

Northwood in Atlantic Canada is seeking an HSW Supervisor to oversee a team of home support workers in a designated geographic area. The role focuses on supervising performance, coordinating care visits, and ensuring high-quality client care.

You will train staff, handle staff and client concerns, and collaborate with Continuing Care and other teams. The position requires LPN licensure, CLPNNS registration, a valid driver’s license, and strong communication, organization, and time-management

Qualifications

• Graduate of an accredited school of Licensed Practical Nurses.
• Active registration with the College of Licensed Practical Nurses (CLPNNS).
• Valid driver's license and reliable vehicle.
• Strong communication, interpersonal, and organizational skills.

Responsibilities

• Monitors the expected outcomes of field staff performance.
• Monitors care delivery in the home by conducting client visits.
• Communicates with clients and family on changing homecare needs.
• Communicates with Continuing care regarding client care needs.
• Responds to concerns and complaints from staff and clients.
• Monitors staff /client activity through Procura reports.
• Attends and participates in meetings with team, committee, boards and clients.
• Provides training and support for staff when required.
• Completes other tasks as required.
