Job summary

apex focus group seeks a Community Connections Manager to lead a warm, inclusive welcome for residents, families, visitors and staff. The role blends mindful presence with relationship-building, storytelling and administrative duties at the Welcome Hub.

The position collaborates with the Human Experience team to celebrate daily moments and support community connections within a Home environment. Competitive wages and benefits are offered.

Qualifications

  • Diploma in Social Services, Community Development, Communications, or Gerontology.
  • Experience in long-term care, senior living, community support, or social services.
  • Compassionate, approachable, and emotionally intelligent communicator.
  • Strong interpersonal and storytelling abilities; comfortable engaging with diverse individuals and families.
  • Excellent organizational and administrative skills with attention to detail.
  • Proficiency with Microsoft/Google suites, basic digital photography or media tools.
  • Must provide Vulnerable Sector Check (CBC) within required timeframe or willingness to obtain one.
  • Provide proof of required vaccines per employer requirements.
  • Two supervisory references required.

Responsibilities

  • Create a warm, welcoming atmosphere for residents, families, visitors and team members.
  • Support residents and families with information, guidance and connection.
  • Weave connections between residents and team members through daily conversations and engagement.
  • Maintain the front welcome hub and lounge, inviting and organized.
  • Collaborate with Programs Manager on social events for the main entrance area.
  • Share stories through internal/external channels and welcome new team members during moves.
  • Respond to inquiries with professionalism and discretion; direct to supports as needed.
  • Maintain visitor logs, deliveries and confidential correspondence.
  • Provide administrative support, schedule care conferences and coordinate resident transportation.
  • Assist with resident move-in and navigation of Home services.
  • Support access to Resident Trust Accounts as needed.
  • Monitor front entrance and Home surveillance for safety.
  • Conduct monthly audits; participate in team huddles and share feedback.
  • Assist in creating Home communiqués and content for website/social media.

The Community Connections Manager reports to the Executive Director and is the welcoming heart of the Home — creating a sense of belonging for every person who enters. The Community Connections Manager ensures that residents, families, visitors and team members feel known, valued, and supported through genuine human connections. This role blends a mindful, engaging and compassionate presence with relationship building, relational storytelling and administrative responsibilities. This role is located at the Welcome Hub.

The Community Connections Manager works closely with the Human Experience Manager, Programs Manager and Employee Experience Specialist (EES) to liven the main floor lounge - helping to celebrate the people, moments, and experiences that make everyday matter.

Wage Rate:

The starting annual salary is $50,000.

Responsibilities:
1. Connection & Human Experience
  • Create a warm, welcoming and inclusive atmosphere for all residents, families, visitors, and team members.
  • Support residents and families with information, guidance and connection, ensuring comfort and trust in their interactions with the Home.
  • Know each resident and team member and purposefully weave connections between residents and team members, building relationships through daily conversations and authentic engagement.
  • Maintain the front welcome hub and lounge, ensuring the space is inviting, organized, and reflective of home and community life.
  • Collaborate with the Programs Manager on a calendar of social events for main entrance area, including art shows, exhibits and community engagement activities
2. Storytelling & Culture Building
  • Partner with the interdisciplinary team to share stories appropriately through internal and external communication channels, including displays, celebration spaces, bulletin boards, etc
  • Welcoming new team members and facilitating residents moving in
3. Resident and Community Support Services
  • Respond to inquiries by phone or in person with professionalism, empathy and discretion, directing individuals to appropriate supports as needed
  • Maintain visitor logs, deliveries, and correspondence accurately and confidentially.
  • Provide administrative support, care conference booking, organizing resident transportation, mailing resident billings, liaising with community partners, office supply inventory, other administrative tasks as assigned
  • Supporting the interdisciplinary team with the resident move-in process
  • Supporting residents and families to navigate the Home’s services
  • Support residents with access to their Resident Trust Accounts as needed
  • Monitor front entrance to ensure resident safety and awareness of entry/exit activity
  • Monitor the Home’s surveillance system
  • Conduct monthly audits as assigned
  • Participate in daily or weekly team huddles to share resident and/or family feedback
  • Share home’s content of events and special moments with the Communication Team for website and social media posts.
  • Manages the Optisign content and functionality
  • Assist in the production and distribution of Home communiqués under the direction of the Executive Director
  • Support facilitation of resident/family satisfaction surveys and team member engagement survey
  • Participate in home-wide continuous quality improvement activities.
  • Other duties as assigned and or indicated in Job Task Inventory
Qualifications:
  • The qualifications needed to join our family are as follows:
  • Diploma in Social Services, Community Development, Communications, or Gerontology
  • Experience in long-term care, senior living, community support, or social services
  • Compassionate, approachable, and emotionally intelligent communicator.
  • Strong interpersonal and storytelling abilities; comfortable engaging with diverse individuals and families.
  • Excellent organizational and administrative skills with attention to detail.
  • Proficiency with Microsoft/Google suites, basic digital photography or media tools.
  • Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one,
  • As a condition of employment, candidates are required to submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements,
  • Provide proof of all required vaccines. The Employer strongly recommends to \"stay up to date\" with the recommended vaccines. Vaccination requirements are subject to changes as determined by the employer.
