Job summary

WoodGreen Community Services in Toronto is seeking a Coordinator, Community Engagement to advance outreach to diverse seniors, foster partnerships, and support grass roots groups funded to deliver health promotion programs.

The role collaborates across programs, tracks progress, develops training and capacity building, and represents WoodGreen with external stakeholders to ensure equitable access and positive community impact.

Qualifications

Responsibilities

• Engage and support grassroots groups funded to deliver services for vulnerable seniors in the east and mid-east Toronto sub regions.
• Create training curriculum covering topics of community health, health promotion, health advocacy, patient engagement, goal setting, and other topics related to community health.
• Develop training programs for community group coordinators in partnership building and system navigation.
• Establish collaborative relationships with community agencies and attend community meetings.
• Assist with planning and development of program activities for grassroots communities.
• Liaise with staff, volunteers, and external partners to provide program information.
• Support group members to contribute their potential, develop leadership, and build long-term capacity.
• Monitor progress reports and data with the Outreach Team.
• Outreach to new community groups and develop a pipeline for funding.
• Maintain contact to ensure progress towards targets and perform site visits as required.
• Resolve conflicts and handle client complaints.
• Integrate anti-racism and community development principles.
• Assist with regular reports and statistics.

Program Overview – Community Care, Seniors Active Living Centres & Outreach

WoodGreen Community Services’ Outreach to Diverse Seniors program funds grassroots groups and communities to deliver programs for seniors to help them reach their health goals, and enable them to age in community. Grassroots groups deliver programs focused on health promotion, outreach, and connection to existing services. The goal of this program is to provide funding and oversight to grassroots groups to address gaps in health service provision and work towards equity in accessing health services in a community setting.

The Coordinator works collaboratively across WoodGreen programs, particularly with the Outreach Team and other Community Care staff, and maintains strong working relationships with external stakeholders, including grassroots organizations, seniors' groups, healthcare providers, hospitals, primary care teams, community health centres, social service agencies, volunteers, and government-funded partners. The role is responsible for fostering collaborative partnerships, promoting equitable access to health promotion initiatives, supporting community capacity building, monitoring program performance, and contributing to data collection, reporting, and continuous program improvement.

What You Will Do

• Engage and support grassroots groups who are funded to deliver services for vulnerable seniors in the east and mid-east Toronto sub regions, including project oversight.
• Create training curriculum covering topics of community health, health promotion, health advocacy, patient engagement, goal setting, and other topics related to community health.
• Develop training programs for community group coordinators in topic areas such as partnership building and system navigation.
• Establish positive collaborative relationships with appropriate community agencies, including attending and speaking at community meetings and other functions.
• Assist with planning and development of program activities for grassroots communities.
• Liaise with staff from other programs, grassroots groups, volunteers, and community agencies (e.g., hospitals, social workers, doctors, health clinics) to provide program information.
• Support members of each group to contribute their full potential, develop leadership skills, engage their community, and build long term community capacity.
• Work closely with the Outreach Team to review and monitor progress reports and data.
• Outreach to new community groups and develop a pipeline of groups for future funding.
• Maintain on-going contact with all parties to ensure progress towards targets ensuring adherence to program requirements and perform site visits as required.
• Resolve conflict situations, including client complaints and disputes.
• Integrate the principles of anti-racism and community development in programs.
• Assist with the preparation of regular reports and statistics.

What You Bring to the Team

• Bachelor’s degree in relevant field (Social Services, Business, Adult Education, etc.)
• Minimum two (2) years of experience and proven track record in facilitation and/or training in a community setting.
• Experience in Project Management/Coordination considered a strong asset.

What Will Set You Apart

• Experience in designing and implementing health training programs with diverse populations.
• Experience in facilitating community engagement and peer-led initiatives with seniors.
• Previous work experience in the non-profit social services sector and/or health sector.
• Solid knowledge of the health and social services sector.
• Understanding of, and sensitivity to, issues facing immigrants and immigrant communities.
• Previous work experience with grassroots groups or community groups is an asset.
• Skills in leading focus group discussions, conducting surveys, and engaging the community.
• Skills in coordinating activities between multiple agencies.
• Confidence and initiative to resolve issues, complaints, and conflicts.
• Excellent interpersonal, verbal, and written communication skills,
• Patience, compassion, and sensitivity to deal with different cultures and levels of ability.
• Excellent organizational and time management skills.
• Strong computer literacy, including database management, report writing, template design, etc.
• Ability to communicate effectively in one or more languages is an asset.
