Job description

JOB DEFINITION:

This role is focused on the overall administrative coordination of all reportable incidents, accidents and grievances that occur within the organization. It is focused on timeliness and accuracy of reports, generating and tracking investigative reports and corrective action plans, and generating trend reports that focus the management team on organizational risks in this area.


EDUCATIONAL REQUIREMENTS:

Bachelor's Degree required; Master's Degree in Social Work preferred


EXPERIENCE REQUIRED:

3 years direct experience with identifying and processing reportable incidents within a regulatory framework, preferably Justice Center/OMH/OASAS/DOH and OPWDD Incident reporting regulations.


DUTIES AND RESPONSIBILITIES:

  1. Initiate reports through the designated organizational process, troubleshooting any issues the team is having with reporting and following through to closure of report in all required systems within the required timeframe
  2. Adjust report templates and relevant documentation as needed to accommodate recommendations, changes in regulatory requirements and requirements for data analysis
  3. Logging, tracking and trending of reports including providing a monthly, quarterly and yearly trend report under the supervision of the Compliance Officer. Analyze incident data to identify patterns and emerging risks.
  4. Coordinating updates and communication with all relevant departments about reporting expectations and regulatory updates
  5. Managing the filing and maintenance of all completed reports, minutes and relevant documents.
  6. Preparing monthly documents for review at the committee or for audit
  7. Schedule routine and special review meetings, write formal meeting minutes, compile monthly Board report
  8. Review all communications from regulatory bodies related to requests for revision of reports, collect all necessary documentation to ensure closure of incidents and submit under the direction of the Compliance Officer
  9. Train staff how to use NIMRs and set up/assist staff with NIMRS access, learn utilize other incident management systems that are updated or newly implemented
  10. Assist with completing investigation related tasks as assigned by the Compliance Officer including investigative reports, interviews, note taking, generate reports, gather and review documents, and perform on-site assessments.
  11. Issue CAPs for investigations and ensure they are properly circulated and finalized.
  12. File completed investigative reports and closing in NIMRS, the WSIR and internal files.
  13. Assist programs with monitoring of open incidents in NIMRS and ensuring closeout
  14. Attend all regulatory updates and ensure they are shared with all relevant staff.
  15. Generate training materials and providing training/presentations, guidance and support to all staff in understanding and following the Incident Management guidelines and policies that apply within their programs.
  16. Work collaboratively with the training department to ensure the Incident Management Curriculum is updated on a regular basis.
  17. Ensure Safety Checks are entered promptly into the NIMRS System following any event
  18. Participates in regular supervision with the Compliance Officer who provides governing oversight for investigations and provide investigative updates, including written notification and follow-up reports, as needed for administrative action
  19. Must have flexible hours and ability to work remotely and on site as needed. Specific in-office/in-person days are determined based on team and program needs
  20. Completes agency mandated training as assigned
  21. Upholds agency standards in professional conduct and ethics
  22. Adheres to the agency's corporate compliance program, including the Compliance Code of Conduct, the NYS and federal false claims acts, and mandatory reporting
  23. Completes corporate compliance training at hire and at least annually
  24. Fully cooperates in all corporate compliance investigations and reviews
  • Identify and implement quality improvement trainings and activities as part of professional development goals
  1. Complete QI Training during onboarding and participate in QI refresher training as needed.
