Reporting to the Program Supervisor, we have one (1) Regular full-time Community Support Worker position available where the successful incumbent will be providing person-centered support to four unique individuals with developmental disabilities, and physical, medical and/or mental health challenges. Lifts, transfers and personal care are a requirement of this position. The successful candidate needs to ensure the individuals have the opportunity for overall personal growth, community inclusion, recreation, potential employment, and assist in maintaining/expanding personal networks with a focus on health. The successful incumbents will work collaboratively with individuals, families, colleagues, and community partners to promote independence, dignity, choice, and inclusion.

Schedule:Monday, Tuesday and Wednesday: 3 p.m. – 11 p.m. & Thursday and Sunday: 3 p.m. – 10:30 p.m.
