The full-time position (35 hours weekly) involves life-skills coaching, service navigation, and collaboration with therapists to support daily living and community integration.

Qualifications

  • Two-year diploma or degree in a social services related field.
  • Minimum of one year of relevant experience working with adults living with mental health issues.
  • Knowledge of psycho-social rehabilitation principles.

Responsibilities

  • Facilitate life skills training including self-care, home management, community management, and social leisure skills.
  • Assist clients in gaining knowledge of available services, how to access them independently, and how to advocate for self.
  • Assist client in gaining skills to acquire appropriate living accommodations.
  • Provide navigation support in environments such as home, grocery stores, banks, etc.
  • Provide updates to the client’s Primary Therapist to fulfill case conferencing per the Individual Services Plan.
  • Advocate/liaise on behalf of the client to address concerns with case managers as required.
  • Refer pertinent issues to the client’s Primary Therapist at Mental Health (e.g., medication, concerns, changes in functioning).
  • Maintain transparency with the client and case manager as part of the discussion.

Job description

Community Support Worker

Our Community LivingSupport program provides one on one support to individuals living with mental health challenges. We focus on the direct needs of individuals, supporting them in realizing their full potential, achieving independence, and pursuing their hopes and dreams.

In this full time(35 hours per week) position, our CommunitySupport Worker will be responsible for supporting clients with severe and persistent mental health issues to live independently within the community. The majority of the work is done with clients in their home and/or within the community.

ACCOUNTABILITIES
  • Facilitating life skills training that includes teaching of self-care, home management, community management and social leisure skills.
  • Assisting clients in gaining necessary knowledge of available services within the community, how to access services independently, and how to advocate for self.
  • Assist client in gaining the skills necessary to successfully acquire appropriate living accommodations.
  • Provide assistance navigating a variety of environments such as home, grocery stores, banks, etc.
  • Provide updates to the clients’ Primary Therapist to fulfill case conferencing expectations as per the Individual Services Plan Agreement.
  • Advocating/liaising on behalf of the client to deal with any concerns/issues at hand, as required.
  • Supporting clients by referring pertinent issues to the client’s Primary Therapist at Mental Health, (e.g. medication, suicidal ideations, family concerns, changes in client’s functioning level).
  • Transparency with the client (if possible) is a vital part of the conversation prior to connecting with the case manager.
QUALIFICATIONS
Education, Training and Experience
  • Minimum of a two-year diploma or degree in a social services related field.
  • Minimum of one year of relevant experience working with adults living with mental health issues.
  • Knowledge of the principles of psycho-social rehabilitation.
Additional Requirements
  • Criminal Record Check is required.
  • Current First Aid and CPR Certificate.
  • Valid and unrestricted Class 5 BC Driver’s License with access to a safe and reliable vehicle.
  • Clear Driver’s Abstract (provided annually).
  • Position requires Union Membership.

Shift/Schedule/Hours per week: Regular FT; 35 hours per week, Monday to Friday 8:30 am – 4:30 pm

Expected Start Date: ASAP

Wage/Salary: Starting at $29.22/hour

Grid Level: 11
