Qualifications

About the Role:

You will work with a team in our Community Access program to support individuals with disabilities, some with complex behaviors and needs. The environment is fast-paced, requiring adaptability to changing plans and situations. Your days will involve attending, participating, or engaging in various activities with the individuals you support, often in the community, outdoors, and traveling across the city to meet, pick up, or drop off clients. Your focus will be on supporting clients' life skills and goals. The individuals may have developmental delays, Autism, ADHD, Borderline Personality Disorder, Cerebral Palsy, Schizophrenia, or other mental health or developmental conditions.

Daily responsibilities include, but are not limited to:

  • Providing medical, technological, environmental, emotional, physical, and behavioral support
  • Attending medical and legal appointments with individuals
  • Administering medication, completing documentation, and performing key worker duties
  • Assisting with daily chores and activities within the community or in group homes
  • Supporting safety and cleanliness of living spaces
  • Completing tasks such as fire drills and hazard assessments
  • Communicating with OPG/Guardians/Family members
  • Advocating, researching, and collaborating for individual support, including hospital visits if necessary
  • Providing support to individuals on ODP (Opioid Dependency Program) or O.A.T
  • Driving to meet individuals or transport them to activities and appointments
  • Learning behavioral support plans to better understand individual needs
  • Completing incident reports effectively

This is a part-time permanent position of 24 hours/week, with a high possibility of extension. The schedule is Monday to Friday, 9:00 a.m. to 5:00 p.m. The starting pay rate is $21.19 per hour, with potential for a raise after the annual evaluation. After a three-month probation period, you will be eligible for cost-shared health and dental benefits and 5 flex days. Mileage reimbursement is $0.50/km, as driving is required.

Minimum Requirements:

  • A Certificate/Diploma in Human Services or Disability sector (equivalencies considered)
  • Valid Class 5 driver’s license, reliable vehicle, and minimum $2,000,000 liability insurance by the time of hire
  • One-year relevant experience in human services or disability sector
  • Comfortable supporting individuals in a swimming pool
  • Support individuals with diverse disabilities and complex diagnoses

Preferred Requirements:

  • Previous training or experience with lifts, transfers, or g-tubes
  • Comfortable supporting individuals around animals/pets
  • Experience working with families, community resources, and building rapport with stakeholders
  • Experience supporting individuals with mental health illnesses, ASD, or addictions is an asset
