Job summary

PACE Independent Living is hiring a Community Resource Navigator in Ontario. The role works with clients, caregivers, and partners to advance independence, navigate health and social services, and coordinate service planning across multiple sites and client homes. It requires travel and some on-call hours.

The position is full-time (40 hours/week), with core hours 8:30am–4:30pm, a staggered start option, and regular travel between PACE locations. A comprehensive benefits package is included.

Qualifications

  • 3 to 5 years of experience in a community-based health, social service, or client support environment.
  • Experience in case management, life skills instruction, RAI-CHA assessments, and community system navigation.
  • Experience supporting adults with physical and complex disabilities.
  • Experience coordinating transitions across care settings.
  • Experience facilitating client-centered planning and decision-making.
  • Experience working with interdisciplinary teams and external community partners, including Ontario Health atHome.
  • Knowledge of AODA standards and Health Service Organization Accreditation Standards.

Responsibilities

  • Independent Living Facilitation & Client Planning, promote independence and self-determination.
  • System Navigation & Community Partnership Development, build relationships with healthcare providers and community partners.
  • Transitions & Service Coordination, manage care transitions and discharge planning.
  • Client Information, Education & Engagement, provide client-focused information materials.
  • Program Quality, Intake & Evaluation, participate in reviews and data collection.
