We are a specialist service thatprovide community mental health for older adults living in Calderdale, WestYorkshire. Due to service development we arecreating a Core team, the primary role of the team will focus on triagingreferrals and completing initial assessments, therefore we are looking torecruit an experienced Registered Mental Health Nurse.
We are looking for individualswho are enthusiastic about working in an innovative, flexible, and creativeway. You need to be committed to working collaboratively with service users,carers and other disciplines to develop a plan of care and recovery.
We have a strong and wellestablished Multi-Disciplinary Team (MDT) who work together to deliver personcentred assessments, interventions, and support.
A full comprehensive inductionwill be provided and the service places staff clinical and managementsupervision, appraisal, support and professional development as a highpriority.

The role will be working withservice users with severe and enduring mental health needs and/or living withDementia. You will need to be proficient in undertaking initial assessments ofservice users, short term intervention work, triage, crisis interventions aswell as making clinical decisions and working collaboratively with partnerorganisations and primary care staff.
Ideally, you will have experienceof working in a community mental health service, but this is not essential astraining will be provided.
A key element of the role will beleadership, the ability to support colleagues with day-to-day caseloadmanagement, including prioritisation of responsibilities in times of highclinical demand.
The base is at Laura MitchellHealth & Wellbeing Centre, Halifax, where case management will be held andthe service operates Monday to Friday 9am until 5pm.
A Full UK driving license and useof own car is essential.
Informal visits are encouragedand welcomed.
This role may beeligible for skilled worker visa sponsorship, subject to the appointedcandidates salary meeting the minimum salary threshold of £38,700 per annumunder current UK government requirements.

Hascontinuing responsibility for the provision of skilled interventions to peoplewith mental health problems in the community.Works within a multi-disciplinary team as a member of aCoreCMHT.
Managementof own caseload in line with C.P.A. procedures and risk management protocols.
Provisionof leadership to the Core Team in respect of psycho-social interventions to peopleand carers.
Supervision and support of other team members.
Participation in service development and clinicalgovernance structures.
Assessesneeds and identifies problems relevant to the care of people referred.
Devises a plan of care in partnership with the personand carers.
Implements the planned programme of care to ensure ahigh standard is achieved.
Reviews the effectiveness of the care provided andwhere appropriate initiates any action.
Advises and supports the person and carers to promotehealth and well-being and to prevent illness.
Recognises situations that may be detrimental to thehealth and well-being of the person and initiates action that may berequired.
Establishes and maintains satisfactory communicationwith all agencies involved in the persons care in both hospital andcommunity.
Functions as a member of a multi-disciplinary team.
Management of enquiries and direct referrals of peoplein accordance with the Core C.M.H.T. Operational Policy and participatesin the team duty rota.
Maintain professional registration and demonstratecommitment to statutory codes of practice and continued professionaldevelopment requirements.
Responsible for acting up in the absence of the teamleader.
Maintainsaccurate patient care records in accordance with professional codes of conductand trust procedures and ensures records are made available when required bythe appropriate agency.
Participatesin the formulation of policies at unit level.
Maintainsaccurate mileage records.
Managescaseload with due economy of cost and time.
Ensuressafe carriage, storage and administration of drugs prescribed (registerednurses only).
Responsiblefor the ordering of stock with due economy.
Producesmonthly statistics in line with trust directives.
Keepsthe Management Team and colleagues informed of developments relevant to thearea of responsibility.
Supervisesstudents and trainees on placement.
Assists in the teaching and in-service training ofstaff.
Is conversant with current trends in clinicalpractice.
Participates and accepts delegated responsibility inresearch and clinical audit as required.
Attends courses or training sessions relevant to theupdating of knowledge and experience.
Promotesan understanding of mental health and well being.
Ensures health and safety regulations are observed.
Attends mandatory training sessions as directed by theTrust.
Participates in the values based induction and theannual appraisal process.
Supports the Trustscommitment to a healthy work-life balance.
