Job summary

Connective in Surrey, BC is seeking an Outreach/Care Worker to support clients referred by funders, assisting their adjustment to life in the community with compassionate, client‑centered planning. The position requires dedication to safety and inclusion within a unionized team.

Work hours are 40 per week, Tuesday to Saturday, 9:00 to 17:00, with a focus on housing, health, and daily living supports. A Diploma in Social Services and crisis counselling experience are preferred; valid driver’s

Qualifications

  • Graduate with a Diploma or Certificate in Social Services and one-year of relevant experience is preferred.
  • Counselling experience in crisis situations.
  • Experience with clients with history of conflict with the law, developmental disabilities and/or psychiatric involvement.
  • Understanding of providing service to people with different learning abilities.
  • Extensive knowledge of community services in Lower Mainland and Fraser Valley as they relate to persons with psychiatric involvement.
  • Experience in assessment, case/care plan development and implementation.
  • Demonstrated ability to deal with potential violence and tailor programs to diverse needs.
  • Excellent written and verbal communication skills in English.

Responsibilities

  • Receives referrals from funders.
  • Researches background material and consults with professionals and family to assess needs and intervention possibilities.
  • Develops mutually agreed intervention and support plans with measurable goals.
  • Addresses daily living needs (housing, nutrition, hygiene, employment, education).
  • Ensures ongoing legal, mental, and physical health issues are addressed in plans.
  • Provides a range of interventions to meet established goals.
  • Interacts with professionals, families, and organizations to ensure client interests.
  • Serves as a resource to other professionals (courts, probation, psychiatric services).
  • Maintains extensive records and monthly progress reports.
  • Participates in staff and resident/staff meetings.
  • Other duties as negotiated.
