Job summary

Saskatchewan Health Authority is seeking a qualified Nurse for La Ronge Health Centre to provide intensive case management for individuals with chronic mental illness. Work is primarily in mental health community clinics and programs in La Ronge.

Responsibilities include collaborating with a multi-disciplinary team, coordinating care, and utilizing knowledge of community resources and treatment programs to support patients in the community setting.

Qualifications

• Bachelor Degree or Diploma in Nursing or Psychiatric Nursing is required.
• CRNS registration or eligibility is required.
• Psychiatric Nursing diploma or Nursing diploma is acceptable; BPN preferred.

Responsibilities

• Provides intensive case management to individuals with chronic and persistent mental illness.
• Services are provided within individual and/or group context; works with a multi-disciplinary team.
