Job Description: Community Health Worker

In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority, patient safety is a priority and a shared responsibility. Continuous improvement of quality and safety is inherent in this position.

As part of a multidisciplinary healthcare team, the Community Health Worker provides personal care assistance and support to clients and their caregivers/families. The goal is to promote and maximize independence in the home setting, including assistance with activities of daily living and delegated tasks, as directed and in accordance with established plans of care.

Additional Requirements

1. Travel may be required; transportation arrangements must meet operational requirements and may involve the use of a personal vehicle. A valid BC Driver's License is necessary.

Qualifications: Education, Training, and Experience

• Home Support/Resident Care Attendant/Health Care Assistant Certificate from a recognized post-secondary program, or an equivalent combination of education, training, and experience.
• Food Safe Certificate.
• Valid BC Driver's License.

Current registration and good standing with the BC Care Aide & Community Health Worker Registry are required.
