Community Health Worker – (Preventive Care Team)

Temporary Full Time Position: 35 hours per week

Contract Period: May 2025 to March 2026

Salary: $28.42 to $34.12 Per Hour

Job Summary

LAMP Community Health Centre is seeking a Community Health Worker focusing on Preventive Care who will be working on the High Priority Communities Strategy project. This is a temporary contract position starting in November 2023 which is primarily focused on providing upstream and proactive health care for high priority communities through locally informed, culturally responsive interventions to improve access to health and wraparound, social service related supports. This role will report to the Project Manager, High Priorities Engagement Team Mississauga.

Primary Responsibilities

  • Provide client-centered screening, consultation and assessment by recognizing the social determinants of health and associated health risks.
  • Collaboratively develop holistic health prevention care plans with clients, and make referrals to internal and external service providers to support in meeting their health and wellness goals.
  • Lead activities related to the development, implementation and evaluation of health prevention initiatives, procedures, best-practices and protocols.
  • Establish efficient referral pathways to community resources, develop outcome measures, and ongoing evaluation of health prevention initiatives.
  • Identify and build relationships with various community partners.
  • Conduct community outreach to raise awareness of the Preventative Care Program, other LAMP CHC and partnering programs.
  • Work collaboratively with community partners to deliver health prevention programs and related initiatives.
  • Act as a key point of contact for external partners regarding health prevention strategies that address behavioural risk factors contributing to chronic disease.
  • Actively participate in interdisciplinary team meetings and integrate health promotion into programs and services across LAMP. • Engage regularly with Ontario Health (OH) and Ministry of Health (MOH) contacts as required to inform this work
  • Provide learning opportunities to stakeholders, students, new hires, and colleagues.
  • Facilitate teamwork and promote group effectiveness.
  • Respect and include people of diverse backgrounds, as well as their perspectives.
  • Build and maintain strong business relationships with partner organizations, participate in outreach to promote LAMP’s programs, services and explore partnership opportunities.
  • All other tasks and duties as assigned by the Program Manager

Required Qualifications

  • 3+ years of relevant experience in community healthcare, health education and program development
  • 2+ years of relevant experience working directly with high priority and at-risk communities, with knowledge of relevant services and community supports in Mississauga and surrounding areas is strongly preferred.
  • Bachelor’s degree in health studies, such as health science, health administration or kinesiology.
  • Creative and passionate with a desire to learn, roll-up their sleeves and lead with a client centred approach and entrepreneurial mindset.
  • Demonstrated problem-solving, organizational, and interpersonal skills with an ability to work effectively and respectfully in a collaborative, inter-professional team environment.
  • Ability to work well under pressure in a fast-paced environment and be adaptable to the changing needs of the organization.
  • Self-directed and detail-oriented with ability to multi-task, organize, prioritize, and meet deadlines.
  • Ability to influence positive outcomes and successfully manage conflicts.
  • Strong problem-solving skills and attention to detail and accuracy.
  • Solid working knowledge and ability to navigate a PC windows environment, including shared drives.
  • Strong skills and experience using Microsoft applications (Word, Outlook, Excel, PowerPoint & Publisher) and navigate the internet/intranet environment
  • Experience and accuracy using database applications (i.e. Electronic Health Records –Nightingale, funder databases, etc.)
