Job summary

LAMP Community Health Centre is seeking a Community Health Worker to improve access to health services through client-centered intake and assessments, group programs, and holistic lifestyle plans. You will coordinate referrals, gather data, and collaborate with interdisciplinary teams to address social determinants of health and promote equitable healthy aging.

Reporting to the Health Promotion & Community Support Manager, the role supports outreach, health education, and partnership-building

Qualifications

• Post-secondary education in a related field.
• 3+ years’ experience in community health and/or outreach and engagement.
• Knowledge of health equity and anti-oppressive practices.
• Strong communication and facilitation skills.
• Familiarity with Etobicoke communities is an asset.

Reporting to the Health Promotion & Community Support Manager, the Community Health Worker will play a key role in improving access to health services and wraparound support by conducting client-centered intake and assessments, facilitating group programs and holistic lifestyle plans, coordinating referrals, and managing data collection and reporting. The successful candidate will also collaborate with interdisciplinary teams and community partners to design, implement, and evaluate strategies that address the social determinants of health of this priority population and promote equitable health outcomes targeted towards healthy aging.

Primary Responsibilities
