Job summary

LAMP Community Health Centre in Peel Region is recruiting a Registered Dietitian to provide nutrition assessment and develop diabetes-management plans for BAC communities within an interprofessional team.

The role requires accreditation with the College of Dietitians of Ontario, community-based experience in diabetes care, and active engagement in education, documentation, and collaborative practice. Opportunity for growth and impact in a diverse community.

Qualifications

• Completion of an accredited dietetics education and practical training program recognized for registration with the College of Dietitians of Ontario.
• Completion of an accredited Dietetic Internship Program; preferably with a community focus.
• Current membership from the College of Dietitians of Ontario; in good standing.
• Hold professional liability insurance in accordance with the requirements set out by the College of Dietitians of Ontario.
• Experience with case management and case coordination.
• Eligibility for membership with the Dietitians of Canada is an asset.

Responsibilities

• Provide nutrition assessment for clients living with pre-diabetes and Type 2 diabetes, taking into consideration client’s level of knowledge, readiness to change, food habits, and socio demographic background.
• Develop individualized nutrition and diabetes-management plans within the RD scope of practice and health care team by integrating information about client’s health status, clinical practice guidelines, and available community resources.
• Be able to identify and prioritize complex health and social issues.
• Ensure complete, accurate, and timely documentation of program activities and medical records.
• Monitor for accuracy, quality, and thoroughness through participation in chart reviews.
• Communicate information clearly and concisely to clients and health care providers on a regular basis.
• Coordinate care with members of the interdisciplinary team and community services.
• Present health education sessions on various topics related to diabetes.
• Identify and build relationships with various community partners.
• Present clinical practice guideline updates to health care providers and clinical partners.
• Provide learning opportunities to students, new hires, and colleagues.
• Maintain accurate client records in the EMR, ensuring consent, privacy and confidentiality requirements are met.
• Track participation, referrals and program outcomes, and contribute to evaluation and funder reporting.
• Expected to participate in LAMP and West Toronto DEP committee work aimed to improve programs and client care.
• Perform other duties as assigned by Manager

About the Black Health and Social Services Hub

LAMP Community Health Centre (LAMP CHC) is partnering with Roots Community Services Inc (RootsCS) and the Canadian Mental Health Association Peel Dufferin (CMHA Peel Dufferin) to design, develop and implement the Black Health & Social Services Hub with and for the Black, African and Caribbean (BAC) communities in Peel Region.

The hub will provide primary health, mental health, addictions and social services programs that will holistically address the physical, mental, social and cultural needs of BAC communities, and seek to eliminatedisparities and inequities.

minimum two years of related clinical experience, preferably in community-based diabetes care.

Certified Diabetes Educator (CDE) or evidence of working toward attaining preferred.

Strong knowledge of diabetes pathophysiology, related laboratory values, and pharmacotherapy.

Intermediate experience and strength in application of motivational interviewing counselling techniques in an adult population.

Good working knowledge of the Diabetes Canada Clinical Practice Guidelines.

Demonstrated ability to teach and facilitate groups using principles of adult learning.

Strong skills in nutritional counselling, evidence-based practice, outreach, health promotion, team work, and problem solving.

First Aid and CPR is an asset and must be kept current.

Valid Driver’s License is an asset.

Knowledge of LAMP Community Health Centre, East Mississauga and/or the South Etobicoke community.
