Job summary

Fraser Health Authority is seeking an experienced Community Health Nurse – Licensed Practical Nurse to work in the BC community setting. You will collaborate with an interprofessional team to manage client caseloads, provide direct care, oversee care plans, and support transitions to primary/community care.

The role emphasizes teaching, self-management, and wellness, with a focus on preventing avoidable hospital admissions and improving quality of life for clients living at home.

Qualifications

• Current practicing LPN registration with BCCNM.
• 1 year of clinical experience with geriatric/adult patients.
• Valid BC Driver's license.

Responsibilities

• Establish therapeutic relationships with clients and families.
• Screen referrals and perform client assessments and care planning.
• Develop and monitor comprehensive shared care plans with the team.
• Provide direct client care and education to promote health.
• Coordinate transitions across care settings and services.
• Participate in quality improvement activities and documentation.
