The Northwest Territories Health and Social Services Authority invites applications for a Community Health Nurse, Primary Care. Qualifications

  • A Nursing Degree, AND
  • Two years of recent nursing experience in an ER setting or within a Community Health Centre or Public Health Unit.

Responsibilities

  • Deliver primary care and community health nursing services, including health promotion, disease prevention, immunizations, and chronic disease management.
  • Assess, plan, implement, and evaluate patient care to support positive health outcomes across the lifespan.
  • Coordinate care for patients with complex health needs through case management, referrals, follow-up, and collaboration with interdisciplinary team members.
  • Monitor and support preventive health screening and ongoing management of acute and chronic conditions.
  • Educate and empower patients and families to make informed decisions about their health and wellness.
  • Provide culturally safe, trauma-informed, patient-centered care while advocating for equitable access to health services.
  • Maintain accurate clinical documentation and contribute to quality improvement and community health initiatives.
  • Collaborate with healthcare providers and community partners to promote coordinated, high-quality care.

Additional Requirements:

  • Current registration with the College of Nurses of the Northwest Territories and Nunavut must be obtained and held thru employment.
