Job summary

Fraser Health Authority is seeking a Community Health Nurse (RN) to work in the community setting, independently assessing and managing a client caseload. You will collaborate with an interprofessional team, promote health and prevent hospital admissions through teaching, counselling and direct patient care.

You will coordinate care across primary and community services, support client transitions, and ensure high-quality care in home and community environments.

Qualifications

• Current practicing registration as a Registered Nurse with the BC College of Nurses and Midwives (BCCNM).
• One (1) year recent related clinical experience assessing and treating complex geriatric and/or adult patients in acute or community settings, including care and discharge planning.
• Valid BC Driver's license and access to a personal vehicle for business purposes.

Responsibilities

• Establishes a therapeutic relationship with clients, ensuring autonomy and dignity in decision-making and care planning.
• Screens referrals, conducts client assessments, and collaborates with the interprofessional team to coordinate care.
• Develops comprehensive shared care plans with the team and ensures smooth transitions to primary/community care providers.
• Provides direct client care and explains care plans to clients and families according to guidelines and best practices.
• Plans and prioritizes client care tasks, delegates to Community Health Workers as appropriate, and provides in-home demonstrations.
