Fraser Health is seeking a casual Community Licensed Practical Nurse to join the Community Transition Housing team in Surrey, BC. The role involves delivering nursing care within community health settings, under supervision where applicable, and adhering to regulatory guidelines.

Reporting to the clinical operations manager, you will assess client needs, educate patients, administer medications, and participate in quality improvement activities while ensuring a safe care environment.

Competenze

• Education: Grade 12 plus graduation from an approved Practical Nursing program.
• Current practicing registration as an LPN with BCCNM.
• CPR Level C or equivalent and First Aid certificates.
• May require valid BC Driver's License and access to a vehicle.

Mansioni

• Assess and monitor client needs including physiological, psychological, socio-cultural and spiritual aspects.
• Provide a supportive environment and contribute to the care plan.
• Plan and prioritize client care and respond to changing needs within scope.
• Educate clients and families to improve understanding of care plans and disease management.
• Administer medications and provide reminders per policy.
• Document care and update relevant information to ensure safety.
