Job summary

Placements24 in Kimberley is seeking a Community Health Worker to engage with residents, promote health education, and provide basic screenings. This hands-on role focuses on reducing health disparities and improving access to care at the community level.

You will educate, screen, and support people, connect them with local clinics, and participate in data collection to inform health programs. Strong communication and independent work are essential.

Qualifications

• Minimum Grade 12; CHW certificate/diploma is advantageous.
• Experience in community-based or healthcare settings.
• Strong knowledge of local health issues and Kimberley resources.
• Excellent communication, interpersonal and listening skills.
• Ability to work independently and build trust in the community.
• Basic literacy and numeracy; accurate record-keeping.

Responsibilities

• Conduct health education sessions on hygiene, nutrition, and disease prevention.
• Perform basic health screenings (blood pressure, glucose) as trained.
• Assist community members in navigating the healthcare system and accessing services.
• Conduct home visits to assess health needs and provide support.
• Collect health data and report on community health concerns.
• Promote healthy behaviors and link individuals to primary healthcare providers and clinics.
