Job summary

Revolve Staffing is seeking compassionate Community Support Workers to deliver in-person supports for adults living in their homes in High Prairie and surrounding areas. You will help participants engage in meaningful community activities, build social connections, and reduce isolation, guided by individualized goals and preferences.

Key responsibilities include supporting community participation, providing companionship, following ISPs, documenting outcomes, and communicating with coordinators,

Qualifications

• Diploma or certificate in a related field (e.g., Health Care Aide, Community Support Worker, Social Services) or equivalent experience.
• Minimum 1 year experience supporting adults with disabilities, seniors, or individuals in continuing care settings.
• Strong interpersonal and communication skills.
• Ability to work independently and follow care plans.
• Valid Criminal Record Check (with Vulnerable Sector Check).
• Valid driver's license and access to a reliable vehicle (preferred).

Responsibilities

• Support individuals to participate in community-based activities.
• Provide companionship and social support during outings.
• Follow Individual Support Plans (ISPs).
• Document activities, engagement, and outcomes.
• Communicate with program coordinators, facility staff, and families.
• Ensure safety, dignity, and respect at all times.
• Report incidents or changes promptly.
• Travel within the service area may be required.
