Job summary

Canadian Mental Health Association Muskoka – Parry Sound (CMHAMPS) is seeking a Contract Case Manager for the COAST team in Bracebridge. You will provide intensive case management, psychosocial rehabilitation, crisis intervention and treatment under supervision to individuals with serious mental illness.

The role requires a Bachelor's degree in a relevant field, experience with mental health, and strong communication skills.

Qualifications

Bachelor’s degree in a human service/health science discipline relevant to the position; equivalency may be considered.
Experience with mental illness, developmental disability, or substance use issues preferred; includes counselling, case management, or crisis intervention.
Ability to manage internal/external relationships with empathy, emotional intelligence and conflict resolution.
Familiarity with the Residential Tenancy Act; Indigenous Cultural Awareness and Education is a requirement; bilingual in French is an asset.

Responsibilities

Carry out case management, psychosocial rehabilitation, crisis intervention and treatment for clients with serious mental illness.
Collaborate with the COAST team and under clinical supervision to plan, develop, coordinate and provide services.
Assess needs, develop care plans, and coordinate treatment and support for clients living with persistent mental illness.
