Job summary

ParaMed Home Health Care is seeking a Clinical Unregulated Care Provider Supervisor to oversee PSWs and HSWs delivering community-based care. The role requires RN/RPN registration and at least 5 years of nursing leadership experience in home health settings.

You will lead clinical teams, ensure compliance with standards, mentor staff, and collaborate with nursing colleagues to meet funder reporting guidelines while prioritizing patient safety and quality outcomes.

Qualifications

  • Current RPN or RN registration with the College of Nurses of Ontario.
  • Minimum of 5 years experience as a practicing Registered Nurse or Registered Practical Nurse.
  • Comprehensive patient health assessment skills are required.
  • Understanding of the principles of community nursing practice.
  • Proven experience leading and managing a team.
  • Knowledge of clinical documentation standards and practice guidelines is required.
  • Excellent organizational, interpersonal, and teaching skills.
  • Commitment to professional development in clinical practice that is relevant to home health care.
  • Effective critical thinking and problem-solving skills.
  • Must possess a valid driver’s license and access to reliable transportation.
  • Able to provide a current, clear Vulnerable Sector Screening (VSS).
  • C.N.A. Certification as a Community Health Nurse (CCHN(C)) is considered an asset.

Responsibilities

  • Supervise and support PSWs and HSWs to ensure practices comply with all legislated standards of practice as well as company policies and procedures.
  • Respond to and advise/resolve concerns from community staff, patients/families, and other team members.
  • Provide ongoing leadership through regular huddles, face to face meetings and one-on-ones.
  • Initiate patient safety incident investigations and documents to ensure timelines and action plans are met.
  • Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.
  • Engage patients in problem solving using a collaborative approach, offering health teaching, building rapport and trust to identify patient health care needs, strengths, capacities, and goals.
  • Engage staff in problem solving, offering education, coaching, and mentoring in resolution of problems.
  • Maintain the knowledge, skills, and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice.
