Job summary

CarePartners is seeking a Clinical Practice Lead in Waterloo-Wellington (Waterloo Branch) to mentor frontline staff and drive clinical education in collaboration with the PPRE team. The role requires RN/RPN licensure, CNO registration, and 2+ years leading clinical practice in the community.

Qualifications

• Regulated Health Professional (RN/RPN) and registered with the CNO.
• 2+ years of community leadership in clinical practice.
• Experience in formal leadership is an asset.
• Palliative care experience is an asset.
• Wound care experience is an asset.
• Knowledge of adult learning principles is an asset.
• Proficient with Microsoft Office.
• Vehicle is required; some travel within Ontario.
• Excellent English verbal and written communication skills; French is an asset.

Responsibilities

• Supports, mentors and coaches frontline clinical staff with ongoing clinical education designed by the PPRE team.
• Collaborates with regional management for Community Health Services in coordination with Ontario Health atHome and OHT.
• Approves and trains staff in functions per Regulated Health Professions Act and CHS policies.
• Maintains quality management procedures within the Nursing team and department.
• Promotes CHS and maintains positive public relations.
• Builds relationships to enhance local capacity and system navigation supports for patient care delivery.
• Utilizes virtual care to support staff and patients in achieving care plan goals.
• Promotes workplace health and safety guidance.
• Performs other duties as required.
