Job summary

ParaMed Niagara-on-the-Lake is seeking a Clinical Planner (RPN/LPN) to onboard new patients, assess needs, implement and evaluate care plans, and coordinate district-wide services.

You will conduct initial safety and living-environment assessments, schedule visits, maintain records, and inform patients about policies and timelines.

Requirements: Ontario RPN/LPN registration, at least two years’ nursing experience, access to a vehicle, and the ability to pass a VPS background check.

Qualifications

• Registration in good standing as an RPN/LPN with provincial bodies.
• Minimum two years’ experience as a practicing nurse.
• Access to a personal vehicle and reliable transportation.

Responsibilities

• Perform the initial assessment of care needs for new patients and develop individual care plans.
• Conduct initial safety assessment of patients' living environments to ensure safe work conditions.
• Perform initial personal care services for new patients.
• Advise Operations on scheduling parameters for ongoing visits based on clinical requirements.
