Job summary

Fraser Health Authority’s Delta Hospital in Delta, BC, is hiring a Full Time Clinical Pharmacist to join the Primary Care Network team. You will provide direct patient care, develop and monitor pharmacotherapy plans, and collaborate with prescribers in a dynamic interprofessional setting.

Applicants must be a registered Pharmacist in British Columbia with at least two years of patient-care experience in primary care. Vehicle access for business and strong communication skills are required.

Qualifications

• Current registration as a pharmacist with the College of Pharmacists of British Columbia.
• Two years of recent patient care experience, ideally in a team-based primary care setting.
• Strong pharmacotherapy knowledge and ability to collaborate with health care teams.

Responsibilities

• Identifies and resolves drug therapy problems in collaboration with patients and the primary care team.
• Provides comprehensive medication management and monitors outcomes and safety.
• Counsels patients and provides education on drug therapy and adherence.
• Supervises students and residents and participates in quality improvement activities.
