Sinai Health is looking for a Clinical Nurse Specialist (CNS) for the Geriatric Emergency Management (GEM) role. The role is located at our Mount Sinai Hospital campus.
Mount Sinai Hospital is part of a network of senior-friendly organizations supported by The Regional Geriatric Program of Toronto (RGP). As a CNS GEM, you are a member of the Emergency Department (ED) leadership team. You will provide a specialized clinical resource in our hospital's ED for assessing and treating risk older adults. You will screen and assess older adults at high risk and co-ordinate care and follow-up with the interdisciplinary team. You will serve as a consultant and in some cases, engage directly with the care providers and/or community supports to develop a plan of care.
In This Role You Will
- Complete assessments to identify acute symptoms, underlying health conditions, physical, functional, emotional and cognitive status, home environment issues and home supports needed for the at-risk older adult in the Emergency Department (ED).
- Make recommendations for care for frail seniors and assists in their implementation within the ED, and other hospital services, including specialized geriatric services.
- Initiate and/or coordinates the implementation of a comprehensive plan of care developed in collaboration with the ED team, the patient and their essential care partners, and other interdisciplinary team members.
- Assist in the development of linkages with community services.
- Educate patients/families/staff both formally and informally regarding the particular needs of the older adult and the specialized care they require in the ED.
- Develop innovative solutions to address opportunities for improved transitions of care between the ED, inpatient units and/or community providers.
- Provide support to the ED in developing a senior-friendly health care environment.
- Participate in performance and statistical reporting requirements.
- Participate in ED quality improvement and hospital-wide initiatives to and improve the care of older adults.
- Engage in other system initiatives in relation to Sinai Health.
- Participate in the RGP's activities and maintain links with other GEM nurses.
- Support and fosters the philosophy, goals and standards of the Department of Nursing
Job Requirements
Mandatory
- Successful completion of a Master's degree in nursing from a CNO accredited academic institution
- NOTE: A baccalaureate degree from a CNO-approved nursing program with a Master's Degree in a related discipline may be considered and applicants currently enrolled in and scheduled to complete their Master's Degree program within approximately the first year of hire may be considered
- Current certificate of registration with the College of Nurses of Ontario (CNO)
- Minimum 3 years of recent experience in Emergency nursing or Acute Medical/Surgical nursing
- Demonstrated expertise in Geriatric Nursing
- Member of Registered Nurses' Association of Ontario (RNAO)
Preferred
- Canadian Nurses Association (CNA) certification in Geriatrics
Skills and Knowledge
- Utilizes and demonstrates a comprehensive theoretical knowledge base and advanced level of clinical competence in the care of patients
- Effective verbal and written communication skills, interpersonal and customer services skills
- Proven experience successfully leading and facilitating change in collaboration with a multi-disciplinary team.
- Excellent organizational and time management skills with the proven ability to manage competing demands.
- Excellent decision making, problem recognition, problem-solving skills, negotiation and conflict resolution skills
- Commitment to collaborative practice
- Ability to work independently and efficiently in a fast-paced, multitasking, dynamic environment.
- Ability to work effectively under stressful conditions
- Ability to utilize information technology
- Demonstrated satisfactory work performance and attendance history
