Job Responsibilities, But Not Limited To -
In your position, you will be responsible and accountable for the following, plus any additional duties and responsibilities your manager reasonably requests of you.
- Assist the prescribing physician to complete patient enrollment; including gathering all pertinent and appropriate information to complete the necessary forms (private and/or provincial insurance) in a concise, organized, and professional manner.
- Liaise and advocate with third-party providers and provincial governments as necessary to determine coverage options.
- Review patient charts (paper and electronic based).
- Ensure patients are scheduled and notify physicians of any requests.
- Complete Welcome Call activities with the patient according to defined scripts.
- Facilitate any required communication with physicians, Patient Support Programs, or other health care professionals involved in the patient’s circle of care.
- Act as a central resource liaison for program stakeholders, which include physicians, nurses, patients, and clinics.
- Ensure patient records are maintained and data-accurate to meet program reporting.
- Maintain confidentiality of client and corporate information and discuss same only with appropriate SRx Health Solutions’ personnel.
- Adhere to SRx Health Solutions and manufacturer Policies and Procedures.
Complete Other Tasks, As Requested. Clinic Specific Needs- Assess and triage patients requiring Opioid Agonist Therapy.
- Assess and evaluate patients for HIV and HCV engagement and treatment.
- Evaluate patients for potential participation in clinical trials and assist Clinical Research Coordinators in research-related procedures.
- Perform clinical procedures including coordination of medications, phlebotomy, ECGs etc.
- Perform Fibroscan testing for liver fibrosis assessments.
- Administer Sublocade injections. Administer immunizations, and regular injections of vitamins or medications.
- Provide clinical support for vaccinations, physical assessments, STD screening/treatment, wound care and other nursing-related duties.
- Provide clinical support to infectious disease specialists and Registered Nurse (RN).
- Provide support to the Registered Nurse (RN) in interacting with community, academic, and governmental partners in promoting the mission of VIDC.
About Your QualificationsLicensure- Registered Nurse (RN) license.
- License must be active and in good standing in the region or state of practice.
Clinical Experience- Several years of nursing experience (often 2-5+) in a relevant clinical setting.
- Specialized experience in the nurse's area of focus (e.g., oncology, critical care, cardiology).
Skills And Competencies- Advanced clinical knowledge in the specialty area.
- Strong communication and leadership skills.
- Competence in evidence-based practice, quality improvement, and patient safety.
