The Clinical Lead - Wound Care advocates for the integrated, seamless, timely, and equitable delivery of high-quality, safe, efficient, and person-centered, evidence-informed skin and wound care to the people of Ontario Health atHome South West, regardless of the setting.

The Clinical Lead-Wound Care will engage and collaborate with hospital, primary, and community care partners to provide clinical assessment and direct care intervention to patients with wounds, in accordance with best practices in the community.

The Clinical Lead-Wound Care ensures integrated wound care delivery across Ontario Health atHome, formulating and executing a coordinated strategy based on leading practices.

Your role is to help patients be healthier at home, while benefiting from our support for professional growth, wellness, and work-life balance. Some elements are currently remote; regular reporting to the office is required.

What will you do?

  1. Advise on and promote wound care practices.
  2. Support development and implementation of best practices, guidance, and tools to enhance patient outcomes and self-management, reducing ER visits/hospitalizations.
  3. Identify educational needs and provide learning opportunities.
  4. Support annual work plans focused on wound prevention and management.
  5. Participate in multidisciplinary and community meetings to provide clinical assessment.
  6. Collaborate within Ontario Health atHome to meet program needs and targets.
  7. Establish and maintain relationships with patients, providers, and partners.
  8. Act as a clinical resource, interpret wound care roles to stakeholders.
  9. Coordinate services across sectors through liaison and participation with partners.
  10. Engage in problem solving and planning to address wound healing delays and service gaps.
  11. Support the development of a high-performing clinical team.
  12. Stay updated on best practices and tools in skin and wound care education.
  13. Partner on policy and procedure development to ensure quality standards.

What must you have?

  • Registered Nurse in good standing
  • Completion of NSWOC certificate or Master’s in wound care
  • 3-5 years relevant RN experience
  • Experience working with diverse individuals and organizations
  • Proven relationship-building skills
  • Strong collaboration skills for inter-disciplinary meetings
  • Effective communication and data interpretation skills
  • Constructive handling of difficult situations
  • Excellent communication, interpersonal, and presentation skills
  • Ability to influence decision-making and guide staff
  • Knowledge of community health care models and professional standards
  • Knowledge of tools and systems like CHRIS
  • Proficiency in MS Office
  • Time management skills
  • Mandatory COVID-19 vaccination
