The Clinical Lead - Wound Care advocates for the integrated, seamless, timely, and equitable delivery of high-quality, safe, efficient, and cost-effective, person-centered, evidence-informed skin and wound care to the people of Ontario Health atHome South West, regardless of the setting.

The Clinical Lead-Wound Care will effectively engage and collaborate with hospital, primary, and community care partners to provide clinical assessment and direct care interventions to patients with wounds, assessing and treating in accordance with best practices in the community.

Some elements of this role may be performed remotely; however, regular reporting to the assigned office location is required.

What will you do?

  1. Advise and promote the development and maintenance of wound care practices.
  2. Support the implementation of evidence-based best practices and tools to enhance patient outcomes and self-management, aiming to reduce ER visits and hospitalizations.
  3. Identify educational needs of partners and staff, providing learning opportunities to promote knowledge application.
  4. Support the development and delivery of annual work plans focused on wound care prevention and management.
  5. Participate in multidisciplinary teams and community meetings related to wound care for clinical assessments.
  6. Collaborate within Ontario Health atHome to meet program needs and targets.
  7. Establish and maintain effective relationships with patients, providers, and partners to ensure quality care.
  8. Act as a clinical resource, interpreting wound care roles to patients, healthcare professionals, and the public.
  9. Coordinate services across sectors through ongoing liaison with partners.
  10. Engage in problem-solving and planning to address wound healing challenges, including delays and gaps.
  11. Support the development of a high-performing clinical team for wound care delivery.
  12. Stay current with best practices and tools in skin and wound care education.
  13. Partner with Patient Services to develop policies and procedures ensuring quality standards.

What must you have?

  • Registered Nurse in good standing with the regulatory body.
  • Completion of NSWOC certificate or Master’s level education in wound care.
  • 3 to 5 years of relevant nursing experience.
  • Experience working effectively with diverse individuals and organizations, preferably in healthcare.
  • Proven ability to develop effective working relationships and build teams.
  • Strong collaboration skills for inter-disciplinary meetings.
  • Effective communication and data interpretation skills.
  • Ability to handle sensitive situations constructively.
  • Exceptional written and oral communication skills.
  • Interpersonal, diplomatic, and presentation skills.
  • Ability to influence decision-making through collaboration.
  • Knowledge of community health care models and professional standards.
  • Familiarity with tools and systems like CHRIS.
  • Knowledge of administrative practices in South West Ontario.
  • Proficiency in Microsoft Office.
  • Excellent time management and prioritization skills.
  • Mandatory COVID-19 vaccination proof prior to start date.
