The Opportunity

Under the direction of the Manager, the Clinical Intake Counsellor is responsible for ensuring that all referrals are accurately collected, assessed, and processed through established intake procedures. They are accountable for directing clients to the most appropriate and least intrusive services, supporting timely access to care, and maintaining compliance with all legislative, regulatory, and organizational standards.

Responsibilities
  • Screening Referrals: Review incoming outpatient mental health referrals to assess urgency, appropriateness, and service fit, using established triage guidelines.
  • Waitlist Support: Maintain regular contact with individuals on waitlists (psychiatry), providing check‑ins, resources, and interim support as needed to ensure engagement and safety.
  • System Navigation: Assist clients in navigating mental health and community services, helping them access appropriate supports and resources, and meeting with partners to discuss flow of patients through mental health services.
  • Assess mental health risk using risk and assessment tools, standards, guidelines and policies and procedures.
  • Support patients presenting as in crisis and respond in accordance with policy and clinical guidelines.
  • Liaison with external service providers as needed and facilitate external referrals as deemed appropriate.
  • Actively participate in the Hospital’s Health and Safety Internal Responsibility System, maintaining accountability for associated responsibilities.
  • Contribute to the enhancement of the patient experience through continuous improvement, learning, and teamwork, with a focus on excellence, innovation and safety.
  • Perform other duties as assigned by manager, which may include but are not limited to group therapy facilitation and individual therapy.
Qualifications
  • Currently registered in good standing with a health regulatory College in the Province of Ontario.
  • Has two (2) years’ professional experience in a Healthcare or Social Service environment.
  • Possesses comprehensive knowledge of the Mental Health Act, Health Care Consent Act and Substitute Decision Maker Act, as well as Mental Health screening and intervention methods.
  • Knowledge of external Mental Health services providers and resources available in the South Georgian Bay region would be considered an asset.
  • Demonstrated crisis intervention, conflict resolution, mediation, and problem‑solving capability.
  • Must strive to continuously provide a safe and supportive environment for patients, staff, physicians, and volunteers.
  • Must adhere to the duties of workers, as stipulated in the Occupational Health and Safety Act.
  • Must have an ability to effectively communicate (written and verbal) in the English language to provide excellent communication with team members, patients and families. Ability to communicate in additional language(s) is considered an asset.
  • Must have basic computer skills.
  • Must be dedicated to professional growth and development.
  • Must be able to perform all bona fide and essential duties of the position, which may include physical demands such as moving/transferring patients, pushing, pulling, bending, etc.
  • Must have demonstrated excellent attendance/performance record.
  • Excellent organization and prioritization skills.
  • Ability to work independently and effectively communicate with other members of the health care team.

Location: Outpatient Mental Health Services

Employee Group: Non-Union

Employment Type: Temporary Full‑Time (approximately 18 months)

Start Date: In line with next Hospital Orientation intake

Hours of Work: Weekdays, 8am to 4pm. Please be reminded that as a Hospital, in order to provide necessary patient care, our operations run 24/7. Therefore, depending on the program for which you are applying, it may be an expectation that you are available to work weekdays and weekends, as well as all shifts – days, evenings, and nights.

Wage: $49.27 – $58.51 per hour plus applicable shift premiums for evenings, nights and weekends.
