Position Summary: The LVN Clinical Coordinator plays a pivotal role in overseeing and optimizing patient care services. Working under the direction of an RN Clinical Supervisor, this role coordinates with and supports the clinical field staff. Responsibilities include coordinating various aspects of care such as MD orders, patient triage, review and approval of supply orders, admission logs management, follow-up, and assigning labs as needed. They are integral in maintaining high standards of care and facilitating a smooth and supportive experience for patients and their families throughout their home healthcare journey.
