CareRx is seeking a Clinical Consultant Pharmacist to ensure continuity of care for seniors through collaboration with healthcare providers. This role will engage in clinical activities while adhering to health service standards and organizational policies. Ideal candidates are registered pharmacists with long-term care experience, strong problem-solving abilities, and excellent interpersonal skills.

Position Summary :
The Clinical Consultant Pharmacist is responsible for maintaining continuity of care for clientele through collaboration between onsite physicians, nursing staff, and other health providers at long term care, supportive living, and other congregate living communities. This position can either part time or full time depending on applicant's availability. The Clinical Consultant Pharmacist is responsible for completing clinical pharmacy activities in alignment with the Continuing Care Health Service Standards, site policies, internal CareRx policies, and the Ministerial Order for Clinical Pharmacy Services.
