Position Summary:

The Clinical Consultant Pharmacist is responsible for maintaining continuity of care for clients through collaboration with onsite physicians, nursing staff, and other healthcare providers at long-term care, supportive living, and other congregate living communities. This position can be either part-time or full-time depending on the applicant's availability. The role involves completing clinical pharmacy activities in accordance with the Continuing Care Health Service Standards, site policies, internal CareRx policies, and the Ministerial Order for Clinical Pharmacy Services.

Your Day-to-Day Responsibilities:

  • Act as the main representative for our contracted operators
  • Provide clinical pharmacy services to patients in alignment with Continuing Care Service Standards, Site Policy, Internal Policies, and the Ministerial Order of Clinical Pharmacy Services
  • Perform annual and quarterly medication reviews, hospital discharge medication reconciliation, Tamiflu prophylaxis prescribing, etc.
  • Develop collaborative practice networks with physicians, nursing staff, healthcare aides, case managers, and other healthcare providers
  • Provide educational in-services as needed
  • Follow and adhere to internal policies and processes established by the Clinical Department
  • Submit clinical service documentation to the clinical administration department within 48 hours of completion
  • Maintain patient and employee confidentiality according to organizational policies and legal obligations
  • Support best practices in medication management processes
  • Adhere to safety standards and guidelines specific to the organization and department
  • Establish and maintain effective working relationships with physicians, other health practitioners, patients, and family members
  • Assist operational activities by supporting service delivery, site-pharmacy communication, and ensuring continuity of care
  • Coordinate payments on account when necessary
  • Provide on-call services within the team rotation
  • Perform other duties as assigned

Qualifications:

  • Registered and in good standing with the Alberta College of Pharmacy
  • Experience in Long Term Care/Assisted Living Pharmacy is an asset
  • Authorization to administer drugs by injection
  • Proven ability to advance clinical assessment and documentation processes
  • Excellent computer skills
  • Ability to establish and maintain effective relationships with healthcare providers, patients, and families
  • Self-directed, organized, and capable of prioritizing tasks with accuracy and dependability
  • Willingness to travel within the Edmonton area (vehicle required)
