Job summary

Primary Care Alberta is seeking an LPN to serve as a Chronic Disease Management Nurse within the Northwest PCN. You will educate patients, monitor disease panels, and work closely with physicians to deliver safe, family-centered care. Eligible for northern allowance and travel reimbursement, supporting service in northern Alberta.

The role requires four years of LPN experience, strong communication, and willingness to travel to rural communities. Connect Care familiarity is a plus.

Qualifications

  • Completion of an accredited practical nurse education program.
  • Active or eligible for registration with CLHA.
  • Current Basic Cardiac Life Support - Health Care Provider.

Responsibilities

  • Provide education to chronic disease patients and monitor care panels.
  • Collaborate with physicians in the Northwest PCN clinic.
  • Develop care plans and teach patients individually and in groups.
