Job summary

Vancouver Coastal Health is seeking a Registered Nurse to work within community care networks in British Columbia, supporting Chronic Disease Management and preventive care. You will assess patients, coordinate care, and liaise with multidisciplinary teams to improve patient outcomes in a client/family centered model.

Role requires valid RN registration, CDM experience, and a driver's license for local travel; telehealth and community clinics are part of service delivery.

Qualifications

• Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
• Three (3) years' recent, related nursing experience in chronic disease/complex illness education or chronic disease management and prevention, experience facilitating change, group process and team building.
• Valid BC Driver's License. Local area travel requires the use of a personal vehicle for which mileage will be reimbursed.

Responsibilities

• Supports physician practices in office redesign to include comprehensive and coordinated case management for Chronic Disease Management (CDM).
• Develops and implements shared care plans accessible to other healthcare providers involved in patient care management.
