Key Responsibilities
  • Deliver patient care on the hemodialysis units in accordance with all hospital policies.
  • Maintain full scope of practice according to the College of Chiropodists of Ontario standards.
  • Deliver preventative and active foot care aligned with Diabetes Canada, Wounds Canada and the International Working Group on the Diabetic Foot guidelines.
  • Implement and deliver periodic foot screening for all chronic hemodialysis unit patients (every 6 weeks).
  • Provide education to patients and caregivers on foot health.
  • Educate the interprofessional hemodialysis unit care team on preventative foot care and the treatment of foot complications.
  • Educate staff so they are aware of the Chiropodist Research Coordinator role.
  • Engage staff and patients in the ENABLE study and provide support and direction on their role in the study protocol.
  • Maintain accurate clinical care records of all patient encounters per hospital policy.
  • Collaborate with the interprofessional team to standardize care plans for patients with active foot complications.
  • Recognize conditions beyond scope of practice and, in consultation, escalate care as needed, including referrals to other health care professionals (e.g., vascular surgery, infectious disease).
Qualifications
  • Undergraduate degree
  • Graduate of a recognized Chiropody or Podiatric Medicine program
  • Current registration in good standing with the College of Chiropodists of Ontario
  • Practicing Chiropodist
  • Experience in advanced wound care
  • Experience working with diverse populations (e.g., seniors, newcomers, LGBTQ+, individuals with chronic and acute conditions, Indigenous peoples)
  • Ability to work independently and as part of an inter-disciplinary team
  • Working knowledge of Virtual Learning Platforms – Zoom and MS Teams
  • Excellent verbal and written communication skills
  • Research knowledge or involvement
  • Experience with electronic medical record systems
  • Ability to travel to satellite hemodialysis unit sites within SHN (Yee Hong, 78 Corporate Drive, Centenary Site)
  • Ability to work 1 evening per week is an asset but not required
