Qualifications

• Must have a current LPN license.
• Experience in geriatric care is preferred.
• Ability to read and write English.
• Ability to work in a team to design and implement care plans.

Responsibilities

• Provide hands-on nursing care for residents in Health Services Neighborhood.
• Oversee, mentor and coach care staff.
• Assess residents' health status and develop individualized service plans.
• Communicate clearly with residents, families and health providers.
• Complete required paperwork with accuracy.
