Job summary

BGC Okanagan in Salmon Arm, BC is seeking a Case Manager for the Youth Outreach Program. You will support vulnerable youth to build resilience, strengthen life skills, and connect with community resources in both office and community settings.

The role requires collaboration with youths, families, the Ministry of Children and Family Development, and other partners to help youth identify goals and plan paths toward independence. A diploma in a related field and valid First Aid are required.

Qualifications

• Diploma in a related human / social service field.
• Or an equivalent combination of education, training and experience.
• Valid First Aid Certificate.
• Class 5 driver’s licence and a clean drivers abstract.
• Successful completion of Agency screening including a Criminal Record Check.

Responsibilities

• Gathers information relevant to youths’ problems, needs and risks by interviewing, observing behaviour, meeting with caregivers and service providers and using inventories and questionnaires.
• Develops and implements family-centred, short-term intervention plans with supervisor input.
• Plans and conducts group or individual sessions using active listening, conflict resolution, and problem solving within a family framework.
• Provides skill building in communication, relationship-building, resiliency, self-esteem, or self-management techniques.
• Evaluates the intervention progress and discusses concerns with the supervisor to reach objectives.
• Outlines services provided and refers to other community resources as required.
• Maintains related records and statistics and provides reports to the supervisor.
• Liaises with youths, families, and other professionals and accompanies them to meetings as required.

BGC Okanagan is seeking a Case Manager for our Youth Outreach Program in Salmon Arm, BC. In this role, you will support vulnerable youth as they build resilience, strengthen life skills, develop healthy relationships, connect with community resources, and prepare for greater independence. You will work collaboratively with participants, the Ministry of Children and Family Development (MCFD), family members, and other natural supports to help youth identify and achieve their goals. Key Duties and Responsibilities:

• Gathers information relevant to the youths’ problems, needs and risks by interviewing, observing behaviour, meeting with caregivers and service providers and using a variety of inventories, checklists and questionnaires. Reviews the information gathered to identify problems, needs and risks.
• Develops and implements family-centred, short-term, issue‑specific intervention plans within program guidelines in consultation with the supervisor.
• Plans, prepares and conducts group or individual sessions using techniques such as active listening, conflict resolution, and problem solving within a family and natural supports framework to resolve the identified problems, needs and risks.
• Provides skill building in areas such as communication skills, relationship‑building, resiliency, self‑esteem, or self‑management techniques as well as systems‑navigation.
• Evaluates the effectiveness of the intervention plan, reports on the youths and their families’ progress, and discusses related concerns with the supervisor to resolve identified problems and move towards defined objectives.
• Outlines services provided by the program and/or organization. Provides information on and referral to other community service providers, resources and professionals as required.
• Maintains related records and statistics and provides reports to the supervisor as required
• Liaises with and/or promotes the interests of the youths and their families with other community service providers, professionals and school personnel as required. Accompanies youth and their families to meetings and appointments as required.

Hours:

28.5 hours per week, Monday to Friday, with some flexibility in hours
