Job summary

Strive Living Society is seeking an Assisted Independent Living Case Manager to join a progressive nonprofit with growing opportunities. The role supports adults with various health and developmental needs to live independently at home and in the community.

You will collaborate with the Director and Program Manager to manage caseloads, coordinate services, and guide participants through transitions while maintaining high standards of client-centered planning.

Qualifications

• Degree in Psychology, Social Sciences, Human Services, Physical Therapy, Occupational Therapy, Behavioural Interventionist/Counsellor, Criminology, Social Work or equivalent.
• Criminal Record Check
• TB Test
• First Aid and CPR Level C
• Valid Class 5 BCDL and reliable vehicle to use for work purposes
• Mandt 2-Day (high-risk clients) - Training provided after hiring

Responsibilities

• Work with the department Director and Program Manager to maintain the program in a fun and flexible environment.
• Coordinate and manage a caseload of assigned responsibilities.
• Act as a resource to individuals served, their family members or representatives and the care providers.
• Oversee contracted community support homes and coordinate health services.
• Collaborate with stakeholders and report to Funders on planning and incidents.
• Assist new participants with transitioning into the program and oversee client-centered service plans.

Work collaboratively with the department Director and Program Manager to maintain the program in a fun and flexible environment. Coordinate and manage a caseload of assigned responsibilities. Act as a resource to individuals served, their respective family members or representatives and the care providers. Perform a variety of professional activities in support of the Program including but not limited to overseeing contracted community support homes, connecting with contracted care providers in the community to oversee service provision, coordinating OT, PT and other health care‑related services to support the individual in service, collaborating with stakeholders, reporting to Funders regarding critical planning and incidents, assisting new participants with transitioning into the program, overseeing individualized client‑centered service plans and ensuring all standards are upheld.

Job Qualifications include:

• Degree in Psychology, Social Sciences, Human Services, Pyhsical Therapy, Occupational Therapy, Behavioural Interventionist/Counsellor, Criminology, Social Work or equivalent.

• Computer literacy and experience with databases


• Excellent written and verbal communication skills
