Canadian Mental Health Association Muskoka - Parry Sound (CMHAMPS) is hiring two Crisis Case Managers to deliver crisis intervention, assessment and short-term support within a multi-disciplinary team in Bracebridge. The role emphasizes community engagement, resources coordination, and empowerment of clients to achieve daily living goals.

The positions require a Bachelor’s degree in a relevant field, crisis intervention experience, and willingness to work varied shifts in a rural Ontario

Qualifications

• Bachelor’s degree in a human service/health science discipline relevant to the position.
• One year of experience with mental illness, developmental disability or substance use issues preferred.
• Experience in crisis intervention, case management or supportive counselling.
• Ability to work autonomously and in a team in a rural setting.
• Familiarity with crisis intervention, crisis resolution and community support; ASIST preferred.
• Bilingual in French/English is an asset; Ontario G or G2 driver's license required.

Responsibilities

• Provides in person/virtual mental health triage, assessment, intervention and short-term support to promote stabilization.
• Assess urgency, risk factors, strengths, and resources to inform crisis intervention and safety planning.
• Assist with coping skills, psychoeducation, advocacy and short-term case management.
• Make referrals to internal/external agencies and liaise with community partners.
• Engage with landlords and community service providers within Muskoka and Parry Sound.

Purpose of the Position: The Crisis Case Manager is responsible for carrying out crisis intervention, case management, and treatment for individuals living with mental illness. This team member has the clinical skills and experience necessary to assess, plan, develop, co-ordinate, and provide treatment by way of supportive services in a multi-disciplinary environment. The Case Manager develops supports and opportunities for clients to increase or develop new skills needed to support success and satisfaction in their daily living, learning, working and social environments with the least amount of professional intervention required. This team member will be an expert in engaging the community and developing a network of landlords, community service providers and clients in the District of Muskoka.

Crisis Case Manager Role:

• Provides in person/virtual mental health triage, assessment, intervention and short-term support to promote the stabilization of individuals presenting with a Mental Health Crisis.
• Assessment of urgency, risk factors, individual strengths,and needed resources/services, informing the individualized provision of crisis intervention, safety planning and treatment within crisis theory and harm reduction approach.
• Assists with increasing or developing new coping skills, provide psychoeducation,advocacy and empowerment, and provide short-term case management.
• Assists with referrals to internal and external community agencies.
• Liaise with community partners including police, hospitals and other community social service agencies within the districts of Muskoka and Parry Sound.

Education and Qualifications:

• Bachelor’s degree in a human service/health science discipline relevant to the position. Equivalency may be considered for a combination of educational experiences. Examples of equivalency include;
  • Mental Health related diploma with relevant work experience PLUS registration with a regulated health professional college
• One year of experience working with persons who have mental illness and/or developmental disability and/or substance use issues preferred, plus experience in providing one or more of the following;
  • supportive counselling
  • case management
  • crisis intervention
• Proficiency in the areas of crisis intervention, crisis resolution, and community support
  • Training in the areas of crisis intervention and crisis resolution including ASIST is preferred
• Manage internal and external relationships, have and demonstrate empathy and compassion, to be aware of emotional intelligence, conflict resolution, and the understanding of the feelings, ideas and beliefs of others.
• Familiarity with the Residential Tenancy Act.
• Indigenous cultural awareness and education is an asset.
• Ability to work autonomously as well as cooperatively within a team environment in a rural setting
• Excellent communication, organization, and interpersonal skills
• Working knowledge of Microsoft Windows, the Internet, and data management software
• A willingness to serve clients within the communities of the District of Muskoka and Parry Sound
• Bilingual in French/English language considered an asset
• May be required to transport staff and/or clients of the Agency and therefore you are to hold a valid Ontario G or G2 driver’s license, have a reliable means of transportation and appropriate insurance for work purposes on your vehicle with a minimum of $1,000,000 in liability coverage.
• Willingness to work days/evenings (including weekends and statutory holidays)

Salary scale:

$29.85 to $39.09 per hour commensurate with qualifications and experience.
