Job summary

Canadian Mental Health Association - Windsor Essex County is seeking a dedicated professional to provide temporary assignment coverage across agency programs. The role reports to the Manager, Clinical Services with program supervision as required.

You will conduct evidenced-based assessments, deliver clinical interventions, develop and evaluate treatment plans, and support recovery through strengths-based care.

Qualifications

• University degree in Social Work (BSW) or Nursing (BScN) required.
• Registration with the College of Nurses or College of Social Workers in good standing.
• Two years recent experience with adult vulnerable populations.
• Knowledge of mental illness, case management, and relevant policy including Mental Health Act.
• Experience administering and interpreting GAD-7, PHQ-9, LOCUS.
• Computer literacy and MS Windows proficiency.
• Bilingualism in English and French is an asset.
• Valid Ontario Driver's License with access to a vehicle and insurance.

Responsibilities

• Identify client needs through ongoing assessment; create treatment plans within 30 days and perform risk assessments.
• Provide education about illness, symptom management, and coping skills.

• Collaborate with clients to implement treatment/care plans focusing on strengths and recovery.
• Develop relapse prevention and crisis intervention plans.
• Advocate for clients and link to internal/external resources; address gaps in essential services.
• Support reintegration into school, work, and social activities; build informal support.
• Provide life skill coaching for Activities of Daily Living and independence.
• Assist clients in accessing financial assistance, housing, and emergency support.
• Engage families and care partners in treatment and recovery.
• Liaise with family and other providers to ensure continuity of care with a least intrusive approach.
• Maintain comprehensive and timely file management and reporting.
• Participate in program evaluation and suggest improvements.
• Act as an Agency Ambassador and stay updated on Mental Health Act and related policies.

Job description

This position provides temporary assignment coverage to a variety of Agency programs as required based on operational needs. The incumbent will report to the Manager, Clinical Services but will have functional supervision by the Program Manager of any agency program for which they are temporarily assigned.

RESPONSIBILITIES & DUTIES:

• Conducting and completing evidenced based assessments, provides clinical intervention to support client needs and ensures objectives of the program assigned to are met.
• Client orientation. Ensuring clients have an understanding of the services being offered as well as an understanding of processes, rights and expectations for participation in the program as well as limits to confidentiality.
• Identification of client needs through ongoing assessment, using tools identified by the agency, based on program requirements. Treatment plans are created with clients within 30 days of entry into program. Risk assessments are completed regularly.
• Providing education about illness, symptom management, and coping skills.
• Developing, delivering and evaluating group educational programs relevant to clients as directed by the manager.
• Working collaboratively with clients to implement treatment/care plans that focus on strengths, wellness and empowerment to support recovery and goal achievement.
• Developing relapse prevention/wellness plans as well as providing crisis intervention.
• Client and Systems Advocacy and Linkage to resources: Assists clients to access information as well as internal/external resources, services or supports which are deemed necessary. Where resources do not exist or they are inadequate, advocates within system to develop or improve essential services and resources for clients.
• Supporting reintegration into school, work and social activities as well as the building of a healthy informal support system.
• Providing life skill coaching to support Activities of Daily Living and independence.
• Assisting clients in accessing financial assistance and benefits.
• Assisting clients in accessing emergency and/or stable safe affordable housing.
• Engaging, educating and supporting families* through the treatment and recovery process and encouraging clients to identify and include a care partner.
• Liaising, when appropriate, with family and other service or health care providers involved with the client to ensure continuity of care using a ‘least intrusive’ approach.
• Ensures all file management and reporting functions are maintained in a comprehensive and timely manner.
• Participates in program evaluation and makes recommendations for changes/improvement.
• Acts as an Agency Ambassador, representing CMHA-WECB in a professional manner
• Maintains up-to-date knowledge of the Mental Health Act and related legislation, the structure and personnel of local social agencies, government departments, institutions and other groups concerned with the client population, and trends and developments in the community that could affect the provision of mental health services.
• Participates in public or targeted educational initiatives as requested by the manager.
• Provides support to volunteers, students and other staff assigned to the program area and substitutes for and assists other agency staff where skills are applicable.
• Able to work in a variety of agency programs, some specialized, and could be called upon to cover at various locations and hours as set in each program.

EDUCATION & SKILLS:

• Requires knowledge and skill acquired through completion of University degree in Social Work (BSW) or Nursing (BScN). Registration with either the College of Nurses or the College of Social Workers in good standing is required
• Two (2) years recent, relevant experience working with adult vulnerable populations in the judgement of the employer
• Comprehensive knowledge of mental illness and treatments; comprehensive knowledge and skills in the functions and principles of case management; up to date knowledge of relevant mental health policy and legislation including the Mental Health Act; knowledge of community resources

• Demonstrated experience administering and interpreting standardized clinical assessment tools, including the GAD-7, PHQ-9, and LOCUS (or comparable evidence-based assessment instruments) is required.
• Computer literacy skills and working knowledge or various Microsoft Windows application is required.
• Bilingualism in both official languages at the advanced level is considered an asset.
• A valid Ontario Driver’s License with access to a reliable vehicle and proof of current insurance and licensing are required for this position.

Hours of Work: The work schedule will be determined based upon program needs. Short-term employees are offered assignments as operations require, flexibility will be required.

The agency’s regular hours of operation are Monday to Friday, 8:30am to 4:30pm. Flexibility of hours may be required to accommodate assignments and to meet client and service needs.

Salary range: Grade 9, $37.84 – $44.81 per hour

Per 2022 - 2026 Collective Agreement

Applicable shift premiums are paid in accordance with the Collective Agreement, Article 17.11.
