Job summary

Canadian Mental Health Association Brant Haldimand Norfolk is hiring a Case Manager for a permanent full-time role based in Haldimand Norfolk with potential later transfer to Brant. The position supports 20+ community-dwelling clients with mental health and related needs, using psychosocial rehab and recovery models.

Requirements include a BA or BSW and relevant experience, strong knowledge of local services, and valid first aid/CPR.

Qualifications

  • Minimum BA or BSW with relevant mental health experience.
  • Solid understanding of community mental health case management and psych-social rehabilitation principles.
  • Significant work experience in a mental health service environment.
  • Current emergency first aid and/or CPR training.
  • Excellent knowledge of local community programs.
  • Strong interpersonal, verbal and written communication skills.
  • Good organizational and computer skills; experience with client data systems (EMH-Ware) and Microsoft Office.
  • Familiarity with psychiatric assessments and instruments (OCAN, GAIN-SS, OPOC) and progress reports.
  • Current criminal records check is required.

Responsibilities

  • Support a caseload of 20+ individuals in Brant, Haldimand & Norfolk Counties, primarily Brant County.
  • Provide community-based mental health interventions using a psychosocial rehabilitation and recovery model.
  • Assist with intake or RAAM duties as needed.
  • Coordinate transportation of clients as required.
  • Maintain accurate documentation and liaise with local programs.

Job description

We are presently accepting applications from qualified individuals to fill the following position in our Community Support Services:

Case Manager
Position: Permanent Full time
Base Location: Haldimand Norfolk entirely for the first 6 months. May move to a split between Brant and HN after 6 months

Start Date: September 2026

Hours of Work: 35 hours per week

Wage Rate: as per CBA

This position provides community focused interventions utilizing a Psychosocial Rehabilitation & Recovery model approach. Reliable transportation is required, and transportation of clients is a necessity. A full job description is available upon request. Hours of work will require some flexibility, but are typically during weekday, daytime hours depending on program and client requirements. Some evening and/or weekend work may be required.

This role will support a caseload of 20+ individuals living in the community who require assistance with mental health issues, concurrent disorders, dual diagnosis, and/or other complex issues within the Brant, Haldimand & Norfolk Counties, but primarily individuals residing in Brant County. Intake or RAAM duties may be included in this position.

A minimum of a BA or BSW and relevant experience is required, with demonstrated familiarity and understanding of Brant, Haldimand & Norfolk mental health & addiction services & system issues.

Additional Preferences & Qualifications:

  • Solid understanding of community mental health Case Management and Psycho-Social Rehabilitation and Recovery principles;
  • Significant work experience in a mental health service environment;
  • Current emergency first aid training and/or CPR training
  • Excellent knowledge of local community programs;
  • Effective interpersonal, verbal and written communication skills and experience;
  • Strong organizational and computer skills; experience in Client Data Systems (EMH-Ware), Microsoft Office;
  • Familiarity with psychiatric assessments and assessment instruments (ie. OCAN, GAIN-SS, OPOC) and progress reports;
