Program Overview – First Step to Home, Housing & Homelessness Services
First Step to Home is a transitional program with housing as an enabler for men 55 years and up who have been street involved and experiencing mental health and/or addictions issues. The programprovides onsite psychosocial primary care supports and services including: intensive case management; harm reduction and addiction supports; therapeutic and supportive counselling; 24-hour staffing; assistance with activities of daily living; nursing services; life skills development; social and recreational activities; volunteer opportunities; and follow up support.

Job Overview
This position offers services to tenants of Supportive Housing Programs, living in their own bachelor style unit. All participants have prolonged mental health and/or addictions; have experienced homelessness and who are working to maintain affordable and sustainable housing. This position is expected to carry out the services in accordance to WoodGreen’s policies and procedures, procedures, Ministry of Health, and health related authorities, and all relevant legislation. This philosophy of this work is in keeping with WoodGreen’s mission to deliver services that promote wellness and self-sufficiency, reduce poverty and inequality, and build sustainable communities.
This position will be responsible for working as part of a team of Client Support Workers, Case Managers, and Clinical Support work who provide case management services, and crisis intervention support.
